UNINSURED SERVICES

PAY FOR THE YEARLY BLOCK FEE FOR PRESCRIPTION RENEWALS WITH A CREDIT CARD

Dr. Richard uses Chargebee to handle a yearly block fee. You can sign up using the link below. The fee is $50 CAD per year per patient. The alternative to paying for the yearly block fee would be to schedule an appointment every three months for a refill.

PAY THE YEARLY BLOCK FEE USING E-TRANSFER, CHEQUE, CASH, OR OTHER METHOD

Instead of using a credit card and Chargebee, you can choose to pay for the yearly block fee by e-transfer, cheque, or cash. In order to do this, please fill out the following form before making the payment. The block fee is $50 CAD per year per patient. Please arrange for this method with Annette before filling out the form.
