I hope you are keeping well. I am just writing to you now to let you know that I will be away this upcoming Monday and Tuesday, June 15th and 16th. 

During this time, I won't be available for calls, texts, or emails. 

I will of course be checking my emails and voicemails after 4PM on Tuesday. 

From the 17th onwards everything will be back to normal.

I hope you are well. I want you to know that I am always on the lookout for new ways to provide you with better care. 

As you know, I take excellent notes when we are discussing your health. But recently the College of Physicians and Surgeons of Ontario made a change to the rules related to the records I keep about your health. 

Now I am required to maintain a Cumulative Patient Profile (CPP). So going forward I will need your help to ensure that we have all of the required documentation for your chart. 

I will be using a software called Ocean by CognisantMD to help me put together your Cumulative Patient Profile. Using Ocean I can send digital forms to your email address and you can easily fill them out at home. 

At the bottom of this email you can see a picture of what the email will look like. Take note the email you receive may look slightly different. 

Ocean is a secure platform and follows Ontario's privacy legislation, PHIPA.

Would you please help me by filling out the forms that I sent you from the platform? I will be sending these emails out to you within the next week or so.

If you have any questions at all, please email me and I would be happy to confirm this request.

Your health, time, and safety is a priority to me, so I've decided to enact a new policy for in-person appointments at my 34 Kenneth Avenue office. Previously, I would leave my side door unlocked and you would be able to freely enter my office. From now on I will be keeping my side door locked, and when you arrive for your appointment I will unlock it for you. All you need to do is ring the video-doorbell at the side of my office and tell me that you've arrived. Then I will buzz you in.

Take note and please avoid coming into the office if you have symptoms of a fever, a cough, or shortness of breath. If you do have these symptoms, please notify me and contact the Middlesex-London Health Unit at 519-663-5317. The best time to show up and ring the doorbell would be around 5 minutes before your appointment starts. If you're going to arrive at my office even earlier than that, please let me know in advance so I can be ready to let you in. I chose to put this new policy into place because of a couple reasons:

Your health With the risk of Coronavirus / COVID-19, it is advised that we limit the interactions that you have with other people. This new policy should help lessen the impact that COVID-19 could have on your health. Since I deal with a wide range of patients, including cancer patients and the elderly, it's best if these at-risk people are not in close-contact with others who may be sick.

Your time Some people have taken to coming into my office during the day without having an appointment booked. I understand the reason for doing this, but unfortunately it takes me away from other patients. This new policy will ensure that I can focus completely on you and your concerns during your scheduled appointment time.

Your security With the new policy, the door will be locked at all times, and I will be admitting patients on an as needed basis. Your security and safety is important to me, and this new policy will prevent strangers from coming into the office.
